When do you need a speech pathologist?

First, I would see a ear, nose, throat specialist (an M.D.) to determine if you have a structural defect that is preventing you from speaking clearly. It does not sound like you have an articulation disorder. I would imagine that a speech path would determine that you do, indeed, have a structural defect and point you towards an ENT
